MOON data forms can be accessed by clicking on the link below. These forms should be taken with you when you run your routes. Please make sure to fill in all of the data at the top of the survey form, as it is important to us for keeping good records. Also, if you are not using playback than you need only fill out the first 6 minutes of blocks on the data forms. Minutes 7 and 8, and 9 and 10 are for Eastern Screech-Owl and Barn Owl playbacks, respectively. If you are a new monitor and are given a randomly selected route, please make note of your exact route stops and the habitat surrounding them within the data forms included below.
