MOON data forms can be accessed via the link below. These forms should be taken with you when you run your routes. Please ensure that you fill in all the data at the top of the survey form, as it is essential for us to maintain accurate records. Additionally, if you are not using playback, you only need to fill out the first 6 minutes of blocks on the data forms. Minutes 7 and 8, and 9 and 10 are for Eastern Screech-Owl and Barn Owl playbacks, respectively. If you are a new monitor assigned a randomly selected route, please note the exact route stops and the surrounding habitat in the data forms included below.
